GMP Network provides physicians within our network who participate with BCBSM PGIP with additional incentives beyond their standard health plan contracts. These incentives are designed to support GMP’s objectives for practice transformation and improvement in key areas, including:

• Quality Metrics – Achieving and maintaining key performance indicators related to patient care quality.
• Utilization Metrics – Efficient resource use to enhance patient outcomes and cost-effectiveness.
• Participation in Quality Initiatives – Engagement in programs such as the Michigan Collaborative for Type 2 Diabetes.
• Care Management Services – Active involvement in coordinated care efforts to improve patient outcomes.
• Educational Engagement – Attendance at educational sessions to support ongoing professional development.

Incentive funding is determined by health plans, with our largest payers—BCBSM, BCN, Priority Health, HAP, and Humana—offering the most significant earning opportunities. GMP leadership establishes incentive metrics annually based on physician performance and guidance from health plans.
