If you believe these services could help you reach your health goals, please talk to your Primary Care Provider (PCP) or Specialist affiliated with GMP Network.

• Let the office staff know that you may have social needs a Community Health Worker can assist with.
• You will be asked to complete a Social Determinants of Health (SDOH) screening to better understand your needs.
• If the screening shows a need, your provider will submit a referral, and a CHW will reach out to begin working with you.

Please note: When a social need is identified, it will be added to your medical problem list. This ensures your PCP can monitor and support you, just like any other condition (such as diabetes).
